About the Program

Ohros's Social Care Network (SCN) program provides health-related social needs to people with certain physical, mental, or behavioral health needs.

•Medicaid recipients must be enrolled in a Managed Care Plan. Child Health Plus, Essential Plan, and Straight Medicaid members do not qualify.

•Additional family members (mother, spouse, or children under 18) may qualify if they demonstrate a need.

•Eligible members must reside in Brooklyn, Manhattan, the Bronx, or Queens (NYC). We do not service Staten Island at this time.

•Initial approval is authorized for up to 6 months. Additional authorization may be obtained if eligible.
