Peer Support and SCN nutrition services are separate programs with their own eligibility and process. Many people start with one and add the other — whichever fits where you are right now.

SCN Nutrition Program
Healthy food, delivered at no cost to you.
Through New York's Social Care Network, eligible Medicaid members receive home-delivered meals, groceries, and kitchen supplies — along with nutrition guidance.
- Up to 3 meals a day, 7 days a week
- Prepared meals or grocery boxes
- Kitchen supplies and nutrition guidance
