Do you qualify?
To receive Enhanced Nutrition Services through New York's SCN program, members must meet the following criteria.
Medicaid Managed Care
Enrolled in a mainstream Managed Care Plan, HIV-SNP, or HARP. (Child Health Plus, Essential Plan, and Straight Medicaid do not qualify.)
Food Insecurity Need
You screen positive for an unmet need related to food access or nutrition.
Priority Health Group
Pregnant or postpartum, high-risk children, chronic illness, SUD, serious mental illness, or high healthcare utilizers.
Clinical Confirmation
For select programs, a medical provider confirms that services are medically necessary for your condition.
Services provided
Eligible individuals and families receive support designed to improve access to nutritious food and essential kitchen resources. Services are provided for each approved member of the household, based on eligibility and program guidelines.
Daily Nutritious Meals
Approved household members may receive up to 3 meals per day, 7 days a week. Choose the option that works best:
- Prepared, ready-to-eat meals
- Raw food & groceries for home cooking
Flexible options — choose what fits your household.
Cookware & Kitchen Supplies
Eligible members may also receive essential kitchen items to help prepare and store food:
- Microwave

Eligible members may also receive nutrition education and guidance to support healthier eating and better health management:

- Meal planning and healthy food choices
